The LeaderShift Project - Community for Practice



An Opportunity to Build Your Leadership Skills

Working closely together, the LeaderShift project and LEADS Canada, have developed a program that is customized for leaders in community and team/community-based primary health care.

LEADS Community for Practice

LeaderShift and LEADS Canada are committed to supporting participants in adopting and sustaining LEADS within their own environments following the completion of programming. This online LEADS Community for Practice (CFP) is available for participants to join, share and discuss their LEADS experiences and resources. Available exclusively to participants accepted into this program, this LEADS Community for Practice will be moderated by a LEADS Coach and Facilitator, and online discussions, shared resources and scheduled events such as webinars will be made available.

In order to access the LEADS Community for Practice as well as to register for the LEADS primer webinar, you will need to create an individual login and password. You will be asked to select your cohort when registering and you will be required to enter the registration code that was included in your program confirmation email.

The Community for Practice is a key resource that will integrate your learning and experiences and is available to participants of the LeaderShift program until March, 2022.


The LeaderShift project is the vision of Community Health Ontario (CHO). This alliance of four provincial associations believes that leaders in community and primary health care are well-positioned to make a critical contribution to an improved and more integrated health system.
